Praise be to ALLAH (The Glorified and Exalted).

1) "Is it true that if you make Du'a (supplication) or read Al-Qur'an for someone that passed away and you love them so much, do they really hear you? And feel you?"

You can find the answer at the following link:
http://islamqa.com/en/ref/14287/graves

2) "Also about the qabar (grave) do they hear what you are saying to them and is there anything wrong with going often to the qabar to visit?"

It was mentioned in a hadith that the deceased can hear the footsteps of the people as they walk away after burial. The fuqaha’ (Scholars) said that it is Sunnah for both the one who passes by and the one who enters the graveyard to say salaams to the inhabitants of the graves. Based on that, the one who is passing by should stop, say salaams, then continue on his way.

It is permissible for men to visit the grave and they may do that often because it reminds them of Death and striving for the Hereafter. Women should not visit the graves. A fatwa can be found on the following webpage:
http://islamqa.com/en/ref/8198/graves

And ALLAH Knows Best.
