How does someone return to Islam?

I will just give brief and simple answer.
The doors of repentance are always open and Allah is greater than we can imagine and unlike his creatures.

He forgives those who return to HIM with pure heart and repentence.
He is most forgiving.

Establish 5 time dialy prayers and find a few good muslim friends and when you have time, read on the life of prophet Muhammad peace be upon him and his companions.

And last, but not least, recite the holy Quran and listen to its recitation when you can.

Salaam brother, I'm slightly confused about your question do you mean someone that was either from a different religion and wants to "revert to Islam" or do you mean someone that used to be Muslim and left the fold of Islam and wants to return again???

Start off by asking for forgiveness and changing ur acts slowly ... if u have strong temptation fasting helps
try collecting knowledge about islam and inshallah Allah (swt) will guide u towards the right path
