Well, it doesn't mean you don't count it if you're giving zakah (since you need to keep a record of it to make sure whether you paid the full yearly due or still have more left to give). It just means you don't boast about it later on - you know, some people might have a bad habit where they give to a poor person/family and then later tell this to others who know those poor people just to look superior in the eyes of others. So the saying means don't boast about it, don't tell others you gave. Make it so secret that if you gave with your right hand, even your left hand is unaware that something was given by its pair.
