Salaat Question pls

Can someone please explain to me the process of fixing a salaat where one is not sure exactly how raka'ats hee/she has performed? In other words, when doubt arouses from whether or not one's salaat is complete or not, what should he/she do? (a link or an explanation will be gladly accepted) thank you

Okay, now I am confused. This is how I learned to do sajda sahw (from a book by Aisha Lemu)...

This is the version of doing it before tasleem:

Two prostrations, tashhahud, the shahadat , and then when you finish saying the shahadat, you do tasleem to the left, and then go into prostration two times. Do the tashhahud again, the shahadat, salat for the Prophet *SAW* and then tasleem.

Now I'm confused.
