Regarding the lips moving, yes it has to in Salah (even is silent prayers).. You can read the fatwa here please :-

Therefore, the lips need to be moving when we are reciting anything in Salah. And in Zohr, Asr (silent prayers), we should not recite so loud that others are disturbed in their recitation. I havent come across what you have mentioned (ie we should be able to hear it with our ears if there is silence). And Allah knows best.
